You must have EvolveSMS installed and have the customization pack in order for this to work.To use this theme go to Evolve Settings -> Theme Settings -> Use Addon Theme -> Chalk & Board Theme.
If look and feel is not same as in the images then try out these ,
Go to settings and
--Set Conversation Headers setting to Theme Default.
--Deselect Custom Background options.
--Deselect Use Device Font option.
--Select Addon Theme Background option.
